St. Francis Manor Earns 5-Star Ratings Across All CMS Performance Categories
St. Francis Manor has achieved a 5-star rating in all categories on the CMS Nursing Home Compare Five-Star Ratings of Nursing Homes as of December 10, 2025. This outstanding achievement is based on the nursing home's performance in three key categories: health inspections, staffing, and quality measures. Among 300 nursing facilities within 200 miles of Grinnell, St. Francis Manor stands out as one of only five facilities to receive 5-star ratings across all three performance categories and the only nursing facility in Poweshiek County to achieve this distinction.
Notably, St. Francis Manor has consistently demonstrated excellence in all three categories for a number of years. Additionally, St. Francis Manor has been recognized nationally as a Top Performing Best Nursing Home for 2026 by U.S. News & World Report and has received a deficiency-free annual survey for the second consecutive year from the Department of Inspections and Appeals. The Hammond Center for Assisted Living has also demonstrated a commitment to excellence, achieving zero insufficiencies on its surveys since opening in 2019.
