Three consecutive years. Five-star care. One remarkable team.

May 30, 2026

St. Francis Manor has once again achieved a deficiency-free annual survey for the third consecutive year during the Iowa Department of Inspections and Appeals’ survey conducted May 11-14.

Additionally, St. Francis Manor has consistently maintained 5-star ratings across all categories on the Centers for Medicare & Medicaid Services (CMS) Nursing Home Care Compare Five-Star Quality Rating System. This exceptional achievement is based on performance in three key categories: health inspections, staffing, and quality measures. Among Iowa nursing facilities within a 200-mile radius of Grinnell, St. Francis Manor stands out as one of only three facilities to receive 5-star ratings across all three performance categories and the only nursing facility in Poweshiek County to currently hold this distinction.

Notably, St. Francis Manor has been recognized nationally as one of the Best Nursing Homes for 2026 by U.S. News & World Report with an overall rating of “High Performing.” As the global authority in health care rankings and consumer advice, U.S. News & World Report evaluated nearly 15,000 U.S. nursing homes and recognized fewer than 19% as “Best Nursing Homes” in “Short-Term Rehabilitation,” “Long-Term Care” or both. In Iowa, 63 out of a total of 396 nursing homes and skilled nursing facilities received a rating of “High Performing” in either Short-Term Rehab, Long-Term Care or both. While in Poweshiek County, only two nursing homes achieved this recognition.

“We appreciate the tireless efforts of our staff as they continue to strive for excellence,” said Executive Administrator Dion Schrack. “Thank you to our department heads for their leadership roles in providing high-quality service to our residents.”

Despite the presence of the state surveyors, St. Francis Manor residents and staff seized the opportunity to celebrate National Skilled Nursing Care Week, May 10-16, with unlimited enthusiasm. This year's “Moments in Bloom” theme blossomed into a vibrant celebration as residents planted their own flower pots, which will bloom throughout the summer. Staff members wholeheartedly participated in a series of events, including Pictionary, wheelchair races, and musical chairs, which sparked contagious laughter and excitement among the residents. The community came together for an ice cream social and concluded the festivities with a staff grill-out.

“It was really fun to watch the staff’s competitiveness and the residents’ excitement with all of the events,” noted Dion. “A huge thank you to the staff who orchestrated this fun week.”
